Juvenile Applications

· If a grandparent or legal guardian applies for coverage on a child, we need a copy of the guardianship documents.

· All children, ages 0 – 17, within the family should be equally insured.

· We do not insure juveniles, ages 0-17, for more than their parents or legal guardians. Parents/Legal Guardians must have life coverage in force when applying for coverage on children. Face amounts should not exceed coverage on the parents.

· We need the completed Juvenile Questionnaire form 9825 to complete our review.
