Motor Vehicular Report ( MVR) Guidelines

A MVR may be requested on life or health applicants with a history of the following:

1. Excessive use of alcohol.

2. Excessive use of prescription drugs or any use of illegal drugs.

3. Admitted DWI or DUI within the last three years.

4. Admitted accidents, reckless driving or multiple speeding tickets within a year.

Underwriting reserves the right to request a Motor Vehicle Report only if or when deemed necessary. A Motor Vehicle Report will be ordered when applying for Accidental Death Benefit (ADB) on all plans.
